
Do I need to complete forms before my first appointment?
Yes. You are required to complete several documents and forms before your first visit. The intake forms will appear in your TherapyPortal.com account.
As a new client you will receive an email invitation to create an account at TherapyPortal.com. Please follow that link. Remember your email (which is your username) and your password so you can continue to access your portal account.
You will also receive an email invitation (separate from the invitation to create your account) to complete your new client forms. You can follow that link in the email to either log in or create your account.
Once you are logged into TherapyPortal.com:
If you are on your computer: at the top of the portal you will see "Documents." If you are on your phone: at the top right of the portal you will see three black horizontal lines. Click on the lines to expand the menu. You will see "Documents."
You will see a list of paperwork & forms that are on your file.
Click "Complete Form" or "Sign" next to any of the forms that have not been submitted.
Answer the form questions or sign as indicated and then click "Submit Completed Form" to finish your document.
